I think they would just die. The holes from the spikes would probably be enough, but if not that then the spirit web from collapsing. Like how the inquisitors die if you remove one key spike, the Koloss are more physically altered then the inquisitors which only have their brains and hearts changed.

I finally got a campaign going with my friends that they can actually do! And I'm signing a contract with the devil to becom ethe antagonist XD. Anybody else agree that being the Antagonist is just way too much fun?

Going along with the fact that they could literally eat dirt and gain enough nutrients from it to sustain their friekish growth, I'd suspect that they are extremely resistant to any sickness.

Also, in multiple places it mentions shards talking about how its easier to connect to those with holes in their soul, and kelsier sorta did that with the insane man in the secret history.
