I think that both Brandon and the asker had Honorblades on the mind, and just mis-spoke/misheard. That happens a few times at various signings. You're thinking so far into one area you don't even realize you used the wrong word
Maybe Pattern realized what was happening and added his own words to it as well? That would account for anything more than just screaming. Or Relis could have just heard death's agony in the screaming and tried to deny it.

The implication on the Radiants/Shardplate was that of the current Radiants, we haven't seen any PoV's of them+Shardplate, so we don't know what is going on there yet. Renarin's wearing some, but we don't get much with him at all, and even less after the point where (presumably) he gets far enough along to hear the screaming (after the duel).

The phrasing is a little weird, but that's exactly what he said. But it's clear enough to mean that Relis heard the sword-spren screaming because Kaladin was touching the blade.
There's a passage where Shallan thinks of her mother and father fighting. Parental fighting can be pretty traumatic for a child, and would be enough to crack her, I think.
