Edit: Described as a "wooden spear". Maybe it's a false alarm? Don't spears usually have metal heads, though?

 

Most likely simply a description of the shaft. As opposed to a spear made fully of metal or something.

Vin was likely not trying to make a Hemalurgic spike, and Ruin probably wasn't free enough to have Vin make a Hemalurgic spike at that time.

Ruin was capable of, and constantly was, sending hallucinations to people while imprisoned. He got Zane to spike himself. She didn't have her earring in at the time, but I don't agree that just because Ruin was still imprisoned that he couldn't provide the necessary Intent.

The most profound thing here is wherever that spear is, depending on the type of metal on it, it has stolen a bit of the Lord Ruler. Maybe enough for some identity shenanigans.

Well Ruin definitely wasn't free when the earing in question was charged into a spike, so it's definitely not too difficult to put intent into a stabby stabby moment.
