Miles was extremely cocky and didn't even worry about his henchmen stabbing him in the back. Which, makes sense at first glance since he could regenerate from practically anything. 

Except, of course, from any aluminum that happened to be logged in his body. Like the bullets his henchmen had. ALUMINUM BULLETS THAT COULD HAVE KILLED HIM.

It seems like a pretty glaring oversight to me that someone as clever and experienced as his would have overlooked this. Did I miss something?

It is possible that he was unaware of this weakness and even if he was, the aluminum would have to get lodged into something really important and then stay there. If it passed through him or only ended up in a semi vital area he would probably live through that to.

I definately see Miles as being an even more OP kandra in the healing world.  He would shrug off anything excluding aluminum even if it was vital.  He was constantly tapping at such a rate he got shot in the face multiple times and grew it back instantly.  I doubt a shardblade to the spike would have much effect unless they kept it sitting in the spine.   I would imagine aluminum being left buried into a 1 shot kill space would do it.  He might be able to heal himself long enough to perform a surgery to remove something from his heart... It would most likely require aluminum staying lodged into the CNS for enough length to keep him well and dead.  Even at that people live with their brains turned off for a long time.  He could burn gold on instinct alone like a pewter arm, likely until some random person were to remove it or he totally ran out of gold reserves.... 

Even MeLaan being torn to shreds in a corridor of death and just waiting for a new set of bones looks awfully mortal compared to Miles.

The dude used an aluminum knife. He'd stab himself in the arm when he needed a place to put it during a fight. He knew about the weakness. I'm convinced you would have to embed a lot of aluminum shrapnel in his brain for an extended length of time to actually kill him. Even if you put an aluminum bullet in his head, he'd just be unconscious but probably still tapping health until someone cuts his head open and takes the bullet out, at which point he'd revive. Even if he can't heal to push the aluminum out, he can still he all the damage around it, and the brain is very good at finding workarounds. Thus, my conclusion that you'd have to separate so much brain tissue with aluminum that he is either unable to keep tapping (and we have no idea what part of the brain sends that signal) or that he merely remains unconscious long enough to burn through his gold stores. It really is easier just to incapacitate or capture him (as actually happened) and then strip him of his goldmines.

I think in general we underestimate just how immortal he was.
