One of the major things I think he still needs to learn is that he needs to be able to let go. He's still too overprotective of his men; they aren't the powerless any longer, and he has to accept that one of the consequences of that is that his men may choose to sacrifice themselves for what they feel is important. In particular, Kaladin has to realize there's a difference his men being in danger unwillingly versus accepting danger to protect others. Syl seems to recognize this in the chasms:

 

 

He can't save everyone...by himself. He has to be able to accept help, and that means that sometimes other people are the ones that put themselves in harms way. Look at what he's doing at the end of WoR; he's taking it completely upon himself to save his hometown, despite Dalinar's actions to warn Alethka in general. He has to learn to let go; that's the part about leadership he still needs to accept.

 

Excellent points. I wonder how you could make that an ideal.

I don't know if that would remove his scar though. But it is a good thought.

He is still a slave of sorts to his conscience.

 

Or perhaps the marks on his head actually mean something else.

 

I initially thought that perhaps because they had scarred over before he started using stormlight, that it was too late to heal them. That is until Lopen started to regrow his arm.

I'd figure some kind of reflection of the Second Ideal:

 

I will lead those that can protect themselves into protecting others.

It makes me wonder if we havent actually seen foreshadowing for the 4th Ideal already.

 

"I shall not browbeat myself when my followers die for a righteous cause!" but no I still think it has to do with teaching other people to lead. Jezrien was the Herald responsible for training mankinds leaders in how to lead, how to be generals. So it makes sense that the Windrunners who had stylised themselves after Jezrien would do the same. Which is why I think Elhokar's request is so interesting.

I think it might be because he refuses to let go of the past. Once he overcomes this and looks towards the future, it will heal.

The lopen has always envisioned his hand there. Like when he says the missing hand is making a rude gesture. This makes me think it has to do with healing by stormlight. But, it could be that healing old wounds has to do with regrowth.
