Firstly, 

I swing my Urgrash (axe end) at the trapdoor. Do I roll an attack?

After that, I think Thyrvor will gravitate towards the tunnel, but if you guys can open the door he'll probably follow. He's not a leader type.

It doesn't seems like I need to roll to hit outside of combat, so I rolled five for damage.

To see if you can get a crit.

The five damage, slashing does not break the trapdoor.
