So I was rereading the WoK and I found one of the quotes that come from the dying people. It said;

"He must pick it up, the fallen title! The tower, the crown, and the spear!"

-Dated Vevahach, 1173, 8 seconds pre-death. Subject: a prostitute. Back ground unknown.

I'm pretty sure that the quote is about Kaladin, but I'm not positive, and I also think the fallen title is referring to the Knights Radiant. The spear is obvious but I wonder if the tower and the crown are from the Kholin family sigil. The sigil is a tower going through a crown. King Elhokar changes his so it is a sword instead of a tower, so I don't think it is talking about the Alethi crown. I just wonder.

It could be a reference to Kal uniting his spear with the Kholin crown and tower but we also discover in WoR that:

Urithiru is a tower. So, that quote might referencing Kal becoming a true KR after Syl became his spear and that he will lead the KR (the crown) from their ancestry home of Urithiru (the tower). That get problematic though since it seems more like Dalinar will have those last two roles.

It could be a simple hint that he becomes a part of the family either legally, or metaphorically. Since you're re-reading the book, I presume you've finished it at least once? I do agree it's one of the more interesting epigraphs from the two sets of medical records. One of them, I've always personally theorized, was his father's current-time patient records, the other being battle medic records. Just remembered that they are probably Taravangian's. Though, the two sets are distinct.
