The Radiant and Spren separate. The bond degrades due to proximity... And that's that. 

My head canon on taking spren off Roshar is that they need to be pulled into the physical first. I don't see why a spren that can manifest as a shardblade would be unable to leave. If the radiant just carried his shardblade the entire way what is the problem?

I think it's going to be more complicated than that. For a dead Shardblade, locked into a physical form sure. For a living spren I don't think it will be that simple. Hell, we have a WoB saying that circumnavigating Roshar with a spren could be problematic.

So one of the things you’ll have to be asking questions and theorizing on is what happens if you try to carry a spren around the planet. What happens to their Cognitive sense, right? So you’re on Roshar, right? So on the Physical Realm what would happen--because on Shadesmar, you have a flattened version. So there are questions for you to be theorizing implicit in that. And one of them is, what happens, you cross a threshold circling the globe, your spren, what happens to them? Right? This relates to the question you’re asking.

Well, two dimensional is the wrong term, but basically.

Having looked it up, they actually asked your question and he didn't directly say whether it would work or not. 

For current era Roshar though, I don't think you could physically take a Sprenblade off world. To worldhop at current tech levels requires accessing Shadesmar. Can Radiants even summon a Shardblade in the Cognitive Realm?

I doubt they can. It seems like the spren have their true form in the Cognitive. I dont think they can change shapes there. It feels likely that they are bound to one form in the CR.
