In RoW, when a group of deadeyes begins gathering near Lasting Integrity, everyone seems to assume that all deadeyes correspond to Shardblades in the physical realm. However, is it possible that some deadeyes are not associated with a Shardblade at all? For example, could deadeyes be created by Radiants who had only sworn the First or Second Ideal? I've read some WoBs about the topic, and it seems that Brandon always assumes that deadeye = shardblade...

It says somewhere in the books that if a shardblade is left alone for long enough, it disappears and only the deadeye remains.

Deadeyes were different. When dismissed as Blades, they returned to Shadesmar to wander. Maya continued to stare as another deadeye walked into the bizarre surf.
“Lost,” Maya whispered. “Those are lost Blades, Adolin.”
Adolin dismounted. “Lost Blades, Maya?”
“Swords,” she said. She still labored sometimes to speak. “In stone. In water. Lost. For so many, many years …”
“What happens to a Shardblade if it’s abandoned?” Shallan asked. “Like if a ship bearing a Shardbearer sinks?”
“It stays there forever,” Adolin said. “Maya, they wouldn’t be here if they’re lost. They’d be manifested as Blades in the real world.”
“No,” she said. “People stop thinking about them. They fade away after centuries … to be lost. Their sword vanishes from your world, and they wander forever.”

I am not sure how much of a Bond the First Ideal actually is. So I doubt that that would create a Deadeye, but I can definitely See a second Ideal Radiant turning their spren into a Deadeye if they broke their oaths

I think a spren who'd formed a Nahel bond but never manifested as a Blade would just become a Deadeye with no corresponding Blade in the Physical world. The Connection to the Physical that they need to manifest as a Blade is created by the advanced Nahel bond.

Well Kaladin nearly broke his Oath in WoR and if I remember correctly the Stormfather spoke about Kaladin killing Syl and At that Point being a Deadeye is he only form of Death the Spren know. 

But I would agree that a 1. Ideal radiant doesnt make a Deadeye if he breaks his oaths
