Well, the Inquisitor was already dead, and had been dead for some time. I imagine he (with his "body") appeared in the Cognitive Realm when and where he died, not when his body was disposed of. I don't think we'd expect to see a corpse move to the Cognitive realm upon it being destroyed, although I admit the case with the Well is a lot different than any conventional form of destroying something.
