He presents it realistically, and while deaths might be surprising, there are always well founded reasons behind it. Attack on Titan kills people left and right just for the shock value (or at least, way more because of that than any other reason). 2

I've never read Game of Thrones but I did read the WIld Card superhero series that he did back in the late 80s and early 90s. He wasn't particularly squemish about bumping off likeable or significant characters in that world either. Often in horrible ways. (e.g. Dinosaur Boy getting literally ripped apart by the Astronomer right next to Dinosaur Boy's father.)
