Apparently the cognitive shadows of threnody do occasionally get out of the system.  They assumed that a problematic spirit must be a threnodite.

The fortress of the Ire was presumably in the space between Scadrial and Threnody. Apparently "the powers of Threnody" wanted to "join the stage".

We see that Kelsier, a Cognitive Shadow was able to make his way that far, presumably the Threnodite Shades could too, unless the Ire were referring to others. We don't know much about Threnody, and that world has had some big changes.

At that time, if we can believe Nazh, Threndodites intentionally made Cognitive Shadows. Native Cognitive Shadows in the Scadrian system were random flukes if tehy existed at all. Hence it looks likely that the Threnodites knew most about Cognitive Shadows and that the vast majority of CSs were from Threnody. The Ire made a reasonable assumption.
