We don't know what would happen, as we haven't seen such a scenario occurring. The closest incident would've been Preservation / Leras.

If the person used a Perpendicularity to enter the Cognitive then they are Physically within the Cognitive Realm. Their death wouldn't be dragged out, it would be similar to getting killed in the Physical. Their Cognitive aspect should still linger on for a while in the same realm. The body might fall back into the Physical.

We don't really know how you kill someone in the CR anyway.  Sure stabbing seems to work but what exactly that means/actually does to you remains kind of a mystery.

You could also drown someone in the beads in Shadesmar. That would kill them, right?

So, that's a RAFO. Because getting stuff out of the Cognitive Realm into the Physical Realm is a different matter from taking stuff from the Physical Realm to the Cognitive.

It's known that people need food and water in the Cognitive realm, so presumably death by starvation is still a concern.  

Hmm, where the Fused that they fought in Shadesmar there physically in their borrowed flesh state, or where they in some sort of a voidspren form?  I got the sense they were there physically which was why they had their normal powers.
