Historically surgery and many advances in medicine grow out of warfare. So I think the first guess is a good one - surgery was a medical discipline because among the warfare minding Alethi it was most often battlefield surgery.

A battle in a city of ... 50-100K with 40K Parshendi and Elhokar, Kaladin, the guards Kaladin knows, the parshendi Kaladin knows, and Moash all manage to show up in a room. Broke suspension of disbelief for me. Way too convenient. Why was Elhokar doing some oaths - why wait until battle? Its just made it silly not tragic. This could have been a great scene but seeing all that improbability....broke me out of it all. Also why not bring the shard plate? Just carry it in a bag. Shallan as is typical of her character got some people killed. No surprise there.
